Possible Causes

The request was received but failed during processing due to a timeout. If using Silent Network Auth, this can happen when the device does not invoke the SNA URL within the timeout period. The default timeout period is 10 minutes.

Possible Solutions

Resend request for this phone number to restart timeout period.
