Twilio couldn't connect to the configured callback URL after all retry attempts, so it couldn't deliver the transcription status event. The transcription itself is unaffected; only the status notification failed.
- DNS resolution failed for the callback host.
- A firewall blocked the connection from Twilio.
- The server behind the callback URL was down.
- Verify that the callback URL resolves and is reachable from the public internet.
- Allow Twilio's IP addresses through your firewall.
