Business Name

Proof of Business Identity

  • Corporate registration document 

The supporting document(s) must have been issued within the last 6 months and be valid at the time of upload

Business Address 
Must be within the locality or region covered by the phone number’s prefix. A PO Box is not acceptable. Use Japanese characters when entering the business address.

If your Corporate registration document does not state your Business Address, please provide one of the following to prove the local Business Address:

  • Certificate of tax payment of the local corporate entity.
  • Utility bill of the local office address.

Note:  A non-forwarding mailer will be sent to verify the locality of the Business Address. 

The supporting document must have been issued within the last 6 months and be valid at the time of upload.

A bank or credit card statement is not a valid utility bill.

Business Purpose

A declaration of purpose (目 的) as shown on the Corporate registration document (履歴事項全部証明書)

 

Proof of Business Purpose

  • Corporate registration document (履歴事項全部証明書) showing declaration of purpose (目 的)   

The supporting document  must have been issued within the last 6 months and be valid at time of upload.

Authorization of Representative

Proof that representative is authorized to act on behalf of the business

Provide one of the following:

  • Power of attorney
  • Corporate registration showing name of authorized representative
  • Letter on the company letterhead under seal showing the person is duly authorized to execute the contract with us

Name of Authorized Representative

Proof of Identity

A valid photo identification is required to verify the authorized representative's name, photo, date of birth, and current address:

  • Driver’s license
  • Certificate of driving record 
  • My Number Card*
  • “Zairyu” residency card (non-Japanese national)
  • Certificate of special permanent resident (non-Japanese national)

*Do not upload the back side of My Number Card

Address of Authorized Representative

Proof of Address

Provide one of the following documents showing current address of Authorized Representative: 

  • Certificate of tax payment

  • Utility bill
